Horizontal forehead lines are one of the most common reasons people ask about Botox at Elume Medspa in Fairfax, VA. This page explains what causes them, how a forehead treatment is planned, and what a visit involves. Every Botox treatment at Elume is planned and performed by Shaheen Ghafari, a licensed Nurse Practitioner. If you want the overview first, start with our Botox treatment page.

What causes horizontal forehead lines?

Horizontal forehead lines come from the muscle that raises your eyebrows. Every time you look surprised, concentrate, or lift your brows to open your eyes a little wider, that muscle contracts and folds the skin above it into creases. When you are younger, the skin springs back as soon as the expression ends. Over years of repetition the creases begin to show at rest as well, and they tend to deepen where the skin is thinnest or where you habitually lift one side more than the other. Sun exposure and skin that has lost some of its elasticity make the lines more visible. Botox addresses the muscle movement behind the lines, which is why it is the usual first treatment for them.

How is Botox planned for the forehead?

The forehead is not treated in isolation. The muscle that lifts your brows works against the muscles that pull them down, the ones behind your frown lines, so relaxing one without considering the other can change how your brows sit. For that reason, Shaheen Ghafari assesses the forehead and the frown lines together and often recommends treating both, even when the forehead is the only area you came in about. She will ask you to raise your brows, frown, and relax so she can see how your muscles move and where the lines actually form. The dose is individual. It depends on how strong the muscle is, how high or low your brows sit, how much movement you want to keep, and whether you have had Botox before. The plan is made at your consultation, not from a chart. If you want to understand how dosing is decided in general, our units guide explains it.

What does a forehead Botox appointment involve at Elume?

Your first visit starts with a free consultation, 30 minutes in person or 15 minutes by phone, where Shaheen reviews your health history, looks at how your forehead moves, and talks through what a realistic result looks like for you. If you decide to go ahead, the treatment itself typically takes 10 to 20 minutes. Your skin is cleansed, a topical numbing cream may be applied if needed, and a series of small injections is placed across the forehead. Most people are in and out within half an hour once the plan is set, and you can return to most of your usual activities afterward, with a short list of things to avoid that Shaheen goes over with you at the visit. Botox is not booked online at Elume. You request an appointment through our scheduling page or call (703) 383-0431, and the office calls you back to confirm a time.

When do results show, and how long do they last?

Botox does not work instantly. Results start to appear within 3 to 5 days as the muscle begins to relax, and settle in about 1 to 2 weeks, which is when you will see the full effect on your forehead. The result lasts approximately 3 to 4 months, after which movement gradually returns and the lines begin to show again. Many people book their next visit around the three month mark to stay ahead of that. Your first treatment is also a calibration: at your next visit, Shaheen can see how your forehead responded and adjust the plan. For more detail, read how long Botox lasts and how long Botox takes to work.

Forehead lines, or frown lines, or both?

Many people who come in about their forehead leave with a plan that also covers the frown lines between the brows, and the reason is anatomical rather than commercial. The forehead muscle lifts the brows; the frown muscles pull them down and together. Relaxing the lifting muscle on its own can let the brows settle lower, which some people notice as a heavier feeling above the eyes. Treating the two groups together keeps the balance. That said, if your frown lines do not bother you and your brows sit naturally high, a forehead-only treatment can be the right call. Shaheen makes that assessment at your consultation. If the lines between your brows are your main concern, read about Botox for frown lines.

Is Botox the right treatment, or is it skin texture?

Botox addresses lines caused by muscle movement. It does not change skin texture, tighten loose skin, or fill a line that has set into the skin from years of folding. If your forehead lines are still visible when your face is completely relaxed, Botox may soften them by stopping the movement that deepens them, but it will not remove a line that has already set. For texture and laxity, a treatment that works on the skin itself, such as Morpheus8, may be the better fit, and some people benefit from both. Our comparison explains the difference: Morpheus8 vs Botox.

Frequently Asked Questions

Will my forehead still move after Botox?

Yes, if that is what you want. The goal at Elume is a forehead that looks rested, not frozen, and the dose is set to soften the lines while keeping natural expression. If you prefer more or less movement, say so at your consultation. We explain why a frozen look happens and how it is avoided in will Botox make me look frozen.

Can I get forehead Botox at my first visit?

The consultation always comes first, in person or by phone. Whether treatment happens at the same visit depends on your health history and the schedule that day, so ask when you book and the office will tell you what to expect.

How often do people come back?

Results last approximately 3 to 4 months, so most people who want to keep the result return three or four times a year. There is no obligation to keep going. If you stop, movement returns and the lines gradually reappear as they were before treatment.

Does forehead Botox hurt?

Most people describe a brief pinch at each injection, and the forehead is one of the less sensitive areas. A topical numbing cream may be applied if needed, and the injections take only a few minutes. We describe the full experience, from the first seconds to three months later, in what Botox feels like.
